What Information we collect & when?
We collect and process information when you phone the clinic to make an enquiry or appointment, when you email us, send an electronic form from our website, and when you attend the clinic in person. We also receive communications from colleagues and multidisciplinary professionals involved in your care.
At the point of enquiry or booking we may ask for:
- Your name
- Your date of birth
- Your address
- Contact telephone numbers
- Your email address
At assessment and subsequent treatment we will ask you for ‘sensitive data’, which will include:
- Your current health
- Your previous health
- Information regarding the condition you are seeking advice about
- Activities you undertake
- Your employment
- Any medication you take
This information will be recorded alongside the findings of your physical examination.
How we store this information
We use PracticePal, a medical cloud-based software system, which uses the latest
encryption technology, to keep your information safe and secure.
Only staff at Core Physio & Fitness Ltd have access to our account.
All computers are Pin or Password protected, and staff have to enter further passwords to log in to the software.
We take all appropriate organisational and technical security measures to protect the information against unauthorised disclosure or unlawful processing.
Any paper work we receive with your information on eg referrals, clinic reports, are kept in locked fling systems.
How we use this information
- Provide a legal record of treatment and advice given to you
- Provide accurate accounting information
- Ensure continuity of care
- Make referrals and communicate with other Health Professionals to ensure your continuing care
- Send you information relating to your treatment eg, exercise programmes
We may:
- Pass information, with your permission, to other medical professionals who may be involved in your care. This may include GPs, Consultants, Occupational Health departments, Podiatrists, Insurance companies or other Health Care professionals
- Use your information for quality feedback purposes
- Use your information for audit
- Most information is passed using email and we take all reasonable precautions to transmit the information securely.
- If we hand you a paper letter, the letter and its contents becomes your responsibility.
We do not pass on your information for commercial purposes and we take all reasonable steps to ensure that the information is kept up to date and rectified as necessary.
It is your responsibility to inform us of any personal information changes.
How long we keep your information.
We have a legal obligation to retain your Clinical records for 8 years after the conclusion of your treatment.
If the record relates to a child or young person the records must be kept until the patients 25th birthday or 8 years after death.
Your rights
You are entitled to a copy of the personal information we hold about you and to have any discrepancies rectified (except those of a valid medical opinion).
